Emerging Use of Therapeutic Hypothermia for Neonatal Hypoxic Ischemic Encephalopathy

چکیده
Objective: Several clinical trials evaluating the efficacy of hypothermia in neonates with encephalopathy have shown significant reduction in death and disability at 18-month follow-up. We sought to: 1) evaluate how the use of hypothermia for neonatal hypoxic ischemic encephalopathy (HIE) is changing in U.S. clinical practice, 2) determine which methods and devices for delivering hypothermia are being used, and 3) evaluate any differences in the course and outcomes of neonates treated with whole body cooling compared with head cooling. Study design: The study comprised a descriptive review of neonates from a network of 330 neonatal intensive care units, reported to the Pediatrix Clinical Data Warehouse (CDW). Multivariable logistic regression was used to calculate the adjusted odds ratios and 95% confidence intervals for variables associated with an increased risk for

Study of Neurodevelopmental Outcomes at 10-14 Months of Age Using Bayley Scale of Infant and Toddler Development in Asphyxiated Newborns with Hypoxic Ischemic Encephalopathy Treated with and without Therapeutic Hypothermia
Background: Therapeutic hypothermia has become an established protocol for all neonates with moderate to severe Hypoxic Ischaemic Encephalopathy (HIE). There are very few studies comparing the neurodevelopmental outcomes in asphyxiated neonates who received therapeutic hypothermia or did not. This study aimed to assess the neurodevelopmental outcomes of asphyxiated neonates with features of HIE...
متن کاملHypoxic-Ischemic Encephalopathy and Serum Magnesium Monitoring and Maintenance.
Magnesium plays important roles in many physiologic functions including protein synthesis, bone development, and cell membrane function. There is some evidence to suggest a role for magnesium sulfate as a therapeutic neuroprotective agent along with therapeutic hypothermia in infants with hypoxic-ischemic encephalopathy, but studies are inconclusive. Ischemic insult and hypothermia may both pla...
متن کاملNeonatal seizures and therapeutic hypothermia for hypoxic-ischemic encephalopathy.
Neonatal seizures are associated with morbidity and mortality. Hypoxic-ischemic encephalopathy (HIE) is the most common cause of seizures in newborns. Neonatal animal models suggest that therapeutic hypothermia can reduce seizures and epileptiform activity in the setting of hypoxia-ischemia, however data from human studies have conflicting results. In this research highlight, we will discuss th...

Introduction & Objective: Hypoxic-ischemic encephalopathy is one of the most important causes of permanent damage to brain tissue that redound to mortality and/or late sequelae such as cerebral palsy or delayed neural development. 15-20 percent of Hypoxic-ischemic encephalopathy (HIE) cases die during neonatal period and 25-30 percent of those who survive suffer from neural development proble...
متن کاملCurrent management of the infant who presents with neonatal encephalopathy.
Neonatal encephalopathy after perinatal hypoxic-ischemic insult is a major contributor to global child mortality and morbidity. Brain injury in term infants in response to hypoxic-ischemic insult is a complex process evolving over hours to days, which provides a unique window of opportunity for neuroprotective treatment interventions. Advances in neuroimaging, brain monitoring techniques, and t...
